A comprehensive review of 84 studies revealed that for generally healthy adults, routine intake of vitamin and mineral supplements provides little to no defense against cardiovascular disease, cancer, or premature death.
Ingesting supplements without a clear necessity might even be harmful. Excessive doses of certain water-soluble vitamins can lead to adverse effects, with high levels of vitamin B6, for instance, being associated with nerve damage. Similarly, an overabundance of fat-soluble vitamins, especially vitamin A, can accumulate and result in toxicity.
A diagnosed deficiency is a valid reason to consider supplementation. Other evidence-based justifications include folic acid consumption before and during early pregnancy, vitamin B12 for individuals unable to obtain sufficient amounts from their diet, and the recommended ten micrograms of vitamin D suggested by UK guidelines for consideration during the autumn and winter months.
Once a legitimate reason for supplementation is established, the timing of intake can occasionally influence absorption. Typically, the concept of “timing” is more related to meals, drinks, and other medications rather than a precise hour of the day.
Iron supplements are commonly used to address iron deficiency anemia. Given that iron deficiency can arise from blood loss and poor absorption, including gastrointestinal conditions, it is crucial to confirm the deficiency and consider its cause rather than self-medicating based solely on fatigue.
Coffee can significantly diminish iron absorption. A small study involving 34 women with iron depletion showed that consuming coffee with a 100mg iron supplement reduced absorption by 54% compared to water. Taking the supplement with breakfast decreased absorption by 66%, despite the meal including orange juice.
The same study indicated that 80mg of vitamin C increased iron absorption by 30%, whereas 500mg provided no additional benefit. It also suggested that absorption was 37% lower in the afternoon compared to the morning. However, this result narrowly missed the conventional threshold for statistical significance, thus not supporting a definitive rule about taking iron in the morning.
Enhanced absorption does not always translate to superior clinical outcomes. In a randomized trial with 440 adults suffering from iron deficiency anemia, iron alone was as effective as iron combined with vitamin C in improving hemoglobin and iron stores.
Calcium can reduce iron absorption during a single meal or dose, especially in large quantities. Nevertheless, a systematic review of human studies found that its impact on long-term iron status was minimal. If you have been advised to take both calcium and iron, consult a pharmacist about whether to separate their intake.
Iron is generally better absorbed on an empty stomach but may cause nausea, constipation, diarrhea, or stomach pain. Taking it with food might make the treatment more tolerable. NHS advice suggests taking ferrous sulfate 30 minutes before eating or two hours afterward where possible, but with or after food if it causes stomach discomfort. Adhering to the instructions for your specific preparation is more beneficial than striving for perfect absorption and consistently missing doses.
Vitamin D is fat-soluble. A randomized study found that vitamin D3 was absorbed more effectively with a meal containing fat compared to a fat-free meal. This supports the recommendation to take tablets or capsules with a main meal. The study compared the fat content of meals, so it cannot determine whether breakfast is superior to dinner.

Certain medications can influence how the body absorbs or processes vitamin D. These include orlistat, a drug used to treat obesity by reducing fat absorption, and corticosteroids like prednisolone, which are used to reduce inflammation. Simply changing the time of supplement intake may not resolve the issue. Continue taking prescribed medications and consult a healthcare professional about whether your vitamin D intake or levels need review.
Vitamin B12 supports red blood cell production and the nervous system and is obtained from animal-derived or fortified foods. A systematic review and meta-analysis, which combined relevant studies, found that vegans who did not supplement had poorer B12 status compared to those who consumed animal products. Supplements improved their blood markers of B12 status.
For most B12 products, the underlying reason for taking them is more significant than the time of day. Metformin and acid-suppressing medications such as omeprazole can reduce B12 absorption or levels over time. Merely separating the doses may not prevent this effect. For prescribed cyanocobalamin tablets, a manufactured form of vitamin B12, the NHS advises taking them on an empty stomach. Follow the packet instructions for over-the-counter products.

When deficiency is caused by poor absorption, treatment may involve high-dose tablets or injections. A review of available clinical trials found low-quality evidence that high-dose oral B12 and injections produced similar short-term changes in B12 levels, so treatment should be selected based on the cause, symptoms, and clinical advice.
Before starting any supplement, ensure you have an evidence-based reason to take it and that the dose is appropriate. Check for the same ingredient in other products, as multivitamins, energy drinks, and “wellness” powders can contain overlapping ingredients. If you use prescribed medications, consult a pharmacist about potential interactions and spacing. Then, follow the label or prescription.
For certain supplements, the surrounding meal or drink can make a noticeable difference. Where no timing effect has been demonstrated, adhere to the recommended dose and instructions at a time you can remember.
